Delete single item KV

DELETE 
/api/projects/:projectId/kv/entry

Deletes one existing KV entry, addressed exactly by scope + key (conversation scope also needs conversationId). The deletion is permanent and cannot be restored; there is no recycle bin for this step. It advances the project storage revision, which is how subscribers such as Desktop drop their mirrored entry. Requires project manage access and kv.manage; a missing entry returns 404.
